What do we need to know when you call about your sick child?

We ask that when you call us you be prepared to tell us how long your child has been ill and what the symptoms are. For example we need to know about:

  • Fever – abnormal temperature
  • Cough
  • Loss of appetite
  • Vomiting
  • Diarrhea
  • Rashes
  • Pain
  • Swelling
  • Other symptoms

Please keep a pen and paper handy to write down any recommendations or medical instructions.

Do I need to be present for my child’s appointment?

We strongly encourage that you are present for your child’s appointments to provide information and required signatures for immunizations. In situations where you must send your child with another adult, a Permission to Treat form is required. Unless a parent or guardian is present, immunizations may need to be deferred.
